This network meeting will discuss the Palliative Care After-Hours Triage Tool and provide an update on Voluntary Assisted Dying.
The Gippsland Region Palliative Care Consortium (GRPCC) have developed the After-Hours Triage tool to support generalist health professionals to deliver a consistent and evidence-based approach to symptom management for clients having palliative care. This interactive session will look at the after-hours triage tool, and how it might be used in the primary health setting by practice nurses to provide information, support and advice to clients/patients. Principles of telephone triage and assessment, and a brief overview of symptom management using the tool will be covered. Session outline:
- Provide information on the After-Hours Project plan
- Consult with participants on what might need to be included in a modified tool for primary health
- Discuss how palliative care looks in general practice and the referral triage tool
- Update nurses on education available in the space of palliative care
Voluntary Assisted Dying 8 Months On – Voluntary assisted dying became legal in Victoria in June 2019. This update will explore some observations from the state-wide Navigator service with a focus on General Practice – what has been working well and what can be improved.
