Our facilitator A. Prof Justin Tse is joined by expert panel members A. Prof Joel Rhee, Magali DeCastro, Roslyn O’Reilly, Meg Rynderman, and Katherine Lane, who share a depth of experience in providing survivorship care in the community context.
This webinar recognises the expert skills GPs and practice nurses bring and the critical role they play in the management of cancer survivors. With cancer survivorship care increasingly akin to other chronic disease management particular attention will be placed on:
Unmet needs, common issues and treatment related late effects
The importance of modifying screening approaches according to treatment related risk factors
Integrating cancer survivorship care with management of comorbid illness
Shared care: PCP with hospital and PCP with community health team members
Funding mechanisms and evidence based resources to support primary care
